I hereby give my full consent and approval to participate in the Forty Plus Footy league. I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as other related activities incidental to my participation, and I am willing to assume these risks.
I hereby certify that I am capable of participating in the designated sport and that I am healthy and have no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as listed below. In addition to giving my full consent for my participation, I do hereby waive, release and hold harmless Forty Plus Footy. With respect to any injury, disability, death, or loss or damage to person or property relating to my involvement or participation in the designated sport and the activities incidental thereto, whether the result of negligence or any other cause.
By clicking the Sign Waiver checkbox you agree to release your image to be used for future marketing material for Forty Plus Footy. My click indicates I have read this release of liability and assumption of risk agreement, I fully understand its terms, I understand that I have given up substantial rights by signing it, and I am signing it freely and voluntarily without any inducement.
The well-being of our players, families, and community is our number one priority. We have developed our health & safety precautions to comply with the current guidelines put in place by the Centers of Disease Control (CDC) and Maryland Department of Health and government recommendations. While we do comply with these guidelines to prevent the spread of COVID-19, we cannot make any guarantees. Our staff are symptom-free, and to the best of their knowledge, have not been exposed to the virus. However, since we are a place of public service/training, other persons could be infected, with or without their knowledge.

Refund Policy

We offer refund and/or exchange within the first 7 days of your purchase, if 7 days have passed since your purchase, you will not be offered a refund and/or exchange of any kind.
Eligibility for Refunds and Exchanges
Your item must be unused and in the same condition that you received it.
To complete your return, we require a receipt or proof of purchase.
Only regular priced items may be refunded, sale items cannot be refunded.
Muscle Gels cannot be returned.

Once your return is received and inspected, we will send you an email to notify you that we have received your returned item. We will also notify you of the approval or rejection of your refund.
If you are approved, then your refund will be processed, and a credit will automatically be applied to your credit card or original method of payment, within a certain amount of days.
Late or Missing Refunds
If you have not received a refund yet, first check your bank account again. Then contact your credit card company, it may take some time before your refund is officially posted.

We reserve the right to change our prices for products displayed at any time, and to correct pricing errors that may inadvertently occur. Warranties & Responsibility for Services and Products
When we receive a valid warranty claim for a product purchased from us, we will either repair the relevant defect or replace the product. If we are unable to repair or replace the product within a reasonable time, the customer will be entitled to a full refund upon the prompt return of the product to us. We will pay for shipment of repaired or replaced products to customer and customer will be responsible for return shipment of the product to us.

The Service and all materials therein or transferred thereby, including, without limitation, software, images, text, graphics, logos, patents, trademarks, service marks, copyrights, photographs, audio, videos, music and all Intellectual Property Rights related thereto, are the exclusive property of FortyPlusFooty LLC.
